HMRC have a free online R&D tax credit template to guide you through a claim. The link is http://webarchive.nationalarchives.gov.uk/*/http://www.hmrc.gov.uk/randd/rd-claim-template-toolbv2.xls
Be careful when using the template as it has not been updated to reflect the latest uplift percentage.
